In order for a student to participate in any team sport, the following forms must be completed.

Free athletic physicals will be offered on Saturday, May 4th. Doctors from AVALA as well as our athletic trainers and coaches will be present to conduct every phase of the physical process. Students must report to the new gym with the attached packet completed and signed by parents and students in order to expedite the process.
